Home & Office Consultation
Home or Office Services
Public health nurses can provide a variety of services in residents' homes or during an office appointment. Nurses can provide:
  • Consultations on a health situation or concern
  • Assessment
  • Health education
  • Referrals to other agencies
  • Community resource information
  • Current information on immunization needs

Residents can find a public health nurse visit helpful for many reasons. Some common reasons include but are not limited to:
  • Birth of a new baby
  • Growth and development assessments of children
  • Communicable disease follow-up
  • Connecting senior citizens to resources to remain independent
  • Lead poisoning case management

Other Options
Public health nurses are not able to diagnose and treat disease. Please consult your health care provider for acute health concerns. If you are under- or uninsured and without a provider, please explore options for the under or uninsured.

For more information or to schedule an appointment or discuss a concern, please contact the Public Health Information and Referral Nurse at (414) 479-8939.
